SAMPLE FORM 6:  Summative Evaluation Plan for "Across Ages" Project


GUIDING EVALUATION QUESTIONS Questions related to immediate outcomes:
  • Do mentored youth show increased self-esteem, social skills, and self confidence?
  • Do mentored youth feel more committed to school?
  • Do youth who visit nursing homes have more positive attitudes about community service?
  • Do parents who participate in social events communicate more frequently with teachers?

Questions related to long term outcomes:

  • Does the program lead to reduced rates of ATOD use?
  • Does the program lead to increased student attendance rates?
  • Does the program lead to improved student school performance?
  • Do students participating in the program have reduced drop-out rates?
MEASURES AND SOURCES OF DATA Immediate outcomes:
  • Student's self report, Hawkins & Catalano School Survey, pre-/post- measured at referral and at the end of the summer.
  • Student's self report of changes in self esteem, Rosenberg Self-Esteem Scale, pre-/post- measured at referral and at the end of the summer.
  • Student's self report of changes in sense of belonging to a community, Sense of Community Scale, pre-/post-measured at referral and at the end of the summer.
  • Student, parent, teacher reported number and opportunity for communication, program developed phone survey, post-only at the end of the school year.

Long term outcomes:

  • Archival indicator data related from school records.
  • School survey data (e.g., items from the Minnesota student survey) administered annually to participating students.
DATA ANALYSIS PLAN Immediate outcomes:
  • Changes in pre-/post-scores of youth.
  • Number and % of parents, youth, and teachers who report an increase in communication.

Long term outcomes:

  • Percent change from previous year on each indicator.
